Waiver and Release of Liability

1. Assumption of Risk

I understand and acknowledge that participation in Anytime Soccer Camps involves inherent risks, including but not limited to:

• Physical injury from soccer activities, running, and contact with other participants
• Injuries from falls, collisions, or equipment
• Weather-related risks including heat exhaustion, dehydration, or sun exposure
• Risks associated with outdoor activities and sports facilities

I voluntarily assume all risks associated with participation in this camp, both known and unknown, even if arising from the negligence of the camp organizers or others.

2. Release of Liability

In consideration of being permitted to participate in Anytime Soccer Camps, I, for myself and on behalf of my child/ward, hereby release, waive, discharge, and covenant not to sue Anytime Soccer Training LLC, its owners, directors, officers, employees, coaches, volunteers, and agents (collectively "Released Parties") from any and all liability for any loss, damage, or injury, including death, that may be sustained by the participant, whether caused by the negligence of the Released Parties or otherwise, while participating in such activity, or while on the premises owned or leased by the Released Parties.

3. Medical Treatment Authorization

I authorize Anytime Soccer Training staff to provide or arrange for emergency medical treatment for my child in the event of injury or illness. I understand that I will be responsible for any costs incurred for such treatment.

I certify that my child is in good physical condition and has no medical conditions that would prevent safe participation in soccer camp activities, except as noted on the registration form.

4. Photo and Video Release

I grant Anytime Soccer Training permission to use photographs, videos, or other media of my child taken during camp for promotional purposes, including but not limited to:

• Website and social media
• Marketing materials and advertisements
• Camp promotional videos

I understand that no compensation will be provided for such use.

5. Code of Conduct

I agree that my child will:

• Follow all camp rules and instructions from coaches and staff
• Treat other participants, coaches, and staff with respect
• Participate safely and responsibly in all activities
• Refrain from bullying, harassment, or inappropriate behavior

I understand that failure to comply with the code of conduct may result in dismissal from camp without refund.

6. Cancellation and Refund Policy

I understand and agree to the following cancellation policy:

• Full refund: Cancellations made 14+ days before camp start date
• 50% refund: Cancellations made 3-13 days before camp start date
• No refund: Cancellations made within 48 hours of camp start date
• Credit toward future camps may be issued in cases of illness or injury with documentation

7. Personal Property

I understand that Anytime Soccer Training is not responsible for lost, stolen, or damaged personal property. I will ensure my child's belongings are clearly labeled.

8. Acknowledgment and Understanding

I have read this waiver and release of liability, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
